Student-Athlete Required Paperwork

 
King's College is committed to assuring a safe and healthy campus and providing students with ongoing quality health care and therefore utilizes electronic medical records (EMRs) via the student health portal to improve our healthcare services at King's College.
 
Required Forms for All Freshmen and Transfer Student-Athletes (Forms are found on the King's student health portal – see how to access below - and in the Required Student-Athlete Paperwork Welcome Letter)

  • Medical History Form (Online Form)
  • King's College Health Insurance Form (Online Form)*
  • Copy of the front and back of your insurance card (Must be uploaded)*
  • Physical Examination (Must be uploaded – click here to access form)
    • Note - Must be completed by your doctor. Physical date must be within six months of your first day of sport activity (for example, first day of team lift or practice is August 28, 2024. Your physical must be completed after February 28, 2024).
  • Immunization Record (Online Form)
  • Athletic Training Orthopedic History (Online Form)
  • Athletic Training Student-Athlete Policies (Online Form)
    • Form includes - review student athlete policies, insurance policy, concussion information/education, review SSI concussion education, football players review heads up tackling)
  • Athletic Training Sickle Cell Form (Online Form)

Required Forms for All Returning Student-Athletes - this must be completed each year. (Forms are found on the King's student health portal – see how to access below)

  • Athletic Training Annual Health History (Online Form)
  • Athletic Training Student-Athlete Policies (Online Form)
    • This form includes review student athlete policies, insurance policy, concussion information/education, review SSI concussion education, football players review heads up tackling.
  • King's College Health Insurance Form (Online Form)*
  • Copy of the front and back of your insurance card (Must be uploaded)*


*Health insurance for student-athletes must be current and include local and sport coverage. Any identified concerns will be reported to the Athletic Director and must be rectified and approved prior to the student-athlete's continuation in intercollegiate sport.
 
King's College refers students to various insurance agents for health insurance options/purchases that are individually structured for students who have no other form of health insurance. REMINDER: Student-athletes who have played at King's College and already have a physical on file do NOT need to have another physical. ALL NEW ATHLETES, if you have a physical, but have not played a sport at King's College the physical must be within 6 months of playing the sport or you will need to have a new physical done. 
 
If you had an injury/illness that prevented you from competing in part of or all of last season/summer season we will need a written clearance from your physician [MD, DO], in order for you to participate.
 
How to access the King's student health portal:
Logon at: https://kingscollege.studenthealthportal.com (you can also go to the King's Athletics' homepage scroll to the bottom and click on the box titled "Student-Athlete Required Paperwork"). You must use your King's College Monarch email user name (with @kings.edu after your name) and password to log on to the portal. If you are new to King's College, you will set up your King's account when you do your on-line placement testing through advisement.
 
Please log on to the portal starting May 13th, 2024 to fulfill the requirements. 
We request all materials to be uploaded by July 17th, 2024, but encourage you to submit your paper work as soon as you can.


Please note; all student-athletes are required to submit their health information online prior to your participation in athletics. Failure to fill out the appropriate forms (Athletic Training Forms and Health Center Forms) by the due date will DELAY your participation in intercollegiate athletics at King's College.

Once forms are submitted to the health portal verifications will be done periodically throughout the summer.
